Dr. Vincent Facchiano
Dr. Facchiano is respected and recognized in the field of Optometry. Since receiving his Doctorate from Illinois College of Optometry in Chicago, Dr. Facchiano has performed clinical studies in the development of new contact lens plastics for the Food and Drug Administration of the United States. He is in the top 10% of Optometrists in the United States and Canada and achieved fellowship with the American Academy of Optometry (FAAO).In addition Dr Facchiano is a Board Certified Optometrist a select distinction held by 1% of all optometrists who parctice in nothern Illinois & southern Wisocnsin. As Diplomate, American Board of Optometry , Dr Facchiano participates in an extensive process that involves completing approved higher education that far exceeds the minimum expectations for state licensure, computer assessments, practice improvement exercises, as well as periodic exams that demonstrate competency to maintain board certification in anterior segment eye disease and contact lens care. He is an active member of the American Optometric Association, current president of IOA’s -Northen Illinois Optometric Society ,Contact Lens Society, the Better Vision Institute, Lighthouse for the Blind, and Guiding Eyes for the Blind. He has also been a mission leader and served in over 25 international eye care missions in developing countries all over world. For the past 32 years Dr. Facchiano has extended his care to those that have the least and need his services the most both at home and abroad. He actively supports local community projects, Red Cross, Rockford Rescue Mission, Habbitat for Humanity and the Jaycees. He is active in such philanthropic groups as Volunteer Optometric Services to Humanity, Gift of Sight Foundation, Medical Mission Tours, One Sight Foundation and Mountain Top Ministry -Haiti. Dr. Facchiano is a licensed Optometrist in Illinois, Minnesota, Texas and Wisconsin. He is the third generation of Optometrists from his family to practice Optometry in the greater Rockford IL and Madison WI area. He is married, is the father of two sons and resides in Boone County.
Dr. Jillian Phelps
Dr. Phelps is extremely enthusiastic about helping patients care for their eyes. Since graduating from Indiana University College of Optometry, she has finely tuned her clinical skills and holds both DPA and TPA certification. Dr. Phelps has extensive primary eye care experience and specializes in pediatric optometry, contact lens fittings, and ocular disease management. As a lifelong learner, Dr. Phelps is committed to continuing her education in the practice Optometry.
She is an active participant in the One Sight program and is a member of the American Optometric Association and Illinois Optometric Association. Dr. Phelp’s dedication to community and giving sight to those is in need, highlights her spirit of giving back.

Melissa is a lifelong resident of Madison WI and has served as a general office manager for over nine years at Dr Facchiano & Associates S.C. and his affiliated offices, Advanced Madison Eye Care LLC. Highly energetic, efficient, and diligent describes Melissa’s management style. What patients and staff admire the most however is the magnetism of her personality and her passion to serve others. Highly trained and educated, Melissa attended local Madison High Schools and Colleges. She remains active in her community and continues her studies towards a Registered Nurse degree. Melissa also serves at St. Mary’s Hospital in addition to managing our office and being a great mother. Doctors and patients seek Melissa out because they benefit from her unique blend of medical and optical training. Melissa adds empathy and attention to detail in her very personalized approach to patient care everyone enjoys.
Dr Jeremy Goetsch –
Meet Dr Goetsch – co owner of our Pearle Vision Center in Madison WI
Dr. Goetsch trained at the Southern College of Optometry, and received his undergraduate degree from the University of Wisconsin and graduated with honors from each institution. He specializes in primary eye care and contact lenses. Dr. Goetsch has energetically served the Madison community for the past three years and has provided community eye care to the disadvantaged. In his free time, he is an active volunteer for American Brittany Rescue.
